Patient forms and initial appointment

As a new patient, you have the opportunity to fill out all forms prior to your initial appointment. Patient forms are available via the button below. If you do not wish to complete forms prior to your appointment, please arrive 15 minutes prior to your consultation to complete.

For your first appointment please bring:

  • Photo ID
  • Your form of payment
  • A complete list of all medications
  • Be prepared to give a full medical/surgical history

When calling to schedule your consultation, please have a credit card number available, so we may hold your scheduled appointment time. We will not run any charges at this time and you are welcome to pay for your appointment with another card or method of payment.

The fee for a consultation is $75. This fee maybe applied to any service/treatment within 60 days of the consultation.

Our goal is to schedule appointments in an efficient and timely manner. Please call us at least 48 hours in advance should you need to cancel or reschedule. All “no-show” appointments will result in a missed appointment/late cancellation fee. Same day cancellations are acknowledged, if emergencies do occur, at the discretion of the practice.

Consultation Process

Consultation time is devoted to understanding your goals and developing an appropriate treatment plan. During your initial consultation we will get to know you, your lifestyle, your skin type, and cosmetic goals. Depending upon scheduling, some treatments can be performed at the time of consultation if no preparation is required. For those treatments that require advanced preparation, we will schedule a time that is convenient for you to return. If you wish to have a procedure performed at the time of consultation due to time constraints or if you are traveling from a distance to see us, we recommend discussing your concerns with our office coordinator at the time of scheduling.

Pricing

It is difficult to provide pricing over the phone, as we believe that proper treatment requires professional assessment and guidance. At the time of your consultation you will be given a written quote and there will be ample time to discuss treatment options and pricing.
